How to Register

 

If you move to the area and live within a six mile radius you can register with our Practice, if you live outside the six mile radius you will need to register with a Practice elsewhere.  You will be asked to complete a registration form for each person registering with the practice. In addition, all patients will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period, while your medical records are transferred from your old practice to this one. We will also need a copy of your photographic ID and one form of proof of address, ie a utility bill or bank statement from the past 3 months. 

Registrations from can be obtained from our reception during opening hours and must be returns to reception fully completed with the required suppoing documents, on a Tuesday or Thursday between 3pm and 4pm.  

If you move out of the practice area (6 mile radius), you will need to register with a new practice as soon as you move.

 

Preference of GP

From the very outset, patients are registered with the practice, not an individual GP. For administrative reasons your medical card will be issued in the name of one of the doctors; however, you can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by-case basis. In this way, not all the doctors in the practice provide all the services offered and specific doctors may not be immediately available at certain time periods, due to demand.

 

Registrations are dealt with on Tuesday and Thursday between 3pm and 4pm 

Non-English Speakers

In order to register with a doctor and obtain a medical card you need to fill in an application form. This form is called an HSCR-1. It is available in a range of other languages. Please click on the respective links below to download the form in translation. The form should be presented to a doctor of your choice in the area in which you reside. This form is also available in any doctors' surgery.
